Lurie Children’s MyChart gives you direct online access to portions of your child’s electronic medical record (EMR), where your child’s health information is stored. Test results, appointment information, medications and more are all securely stored for easy retrieval. This information is safe from unauthorized access since Lurie Children’s MyChart is password-protected and delivered via an encrypted connection. Lurie Children’s MyChart shows you some of the same information your child’s doctor sees. It also provides convenient methods of communication with the doctor’s office. You can request an appointment, request a renewal of prescriptions or send a message — all online.

Information viewable in MyChart is safe from unauthorized access because MyChart is password protected and delivered via an encrypted connection. What are restrictions on viewing my child’s records in Lurie Children’s MyChart?
A patient must be age 18 or older to become a MyChart account holder. Parents or guardians of children under 18 years of age may become an account holder via a request for proxy access to their child’s account by completing the online registration and submitting an authorization form if required.
Caregivers of children ages 12-17 can now sign up for an account without a consent form to sign. This access allows a caregiver the ability to make appointments, message the care team, and check general health information. For more functionality, such as viewing notes and test results, state and local laws require authorization from the minor child (aged 12-17) To regain access to the medical record functionality please submit a signed authorization form: MyChart Proxy Access Authorization Form. (requires your child's signature)
Once a minor patient reaches age 18, MyChart access to the proxy account holder will be automatically revoked, and the patient may set up his or her own account. If the patient is 18 years of age or older and wishes for a parent or guardian to become, or remain, a proxy account holder, the patient and the proxy must complete an authorization form.

We require all patients to have a MyChart account to avoid delays in care related to results, refills and messaging. We also use MyChart as our telemedicine platform, to send you forms, instructions, and much more. We appreciate that the process for accessing the medical records of a minor is more complicated than the creation of most online accounts, and thank you for taking the time to help us work well for you.
