We accept most major insurance plans, and many smaller plans not pictured. Accepted insurance plans may change. If you are a new patient, or if your insurance changes, please get in touch with our office to confirm if your insurance is accepted. Note that words like "choice" and "select" can change insurance coverage despite the overall "brand" of insurance.

Caregivers of children ages 12-17 can now sign up for an account without a consent form to sign. This access allows a caregiver the ability to make appointments, message the care team, and check general health information. For more functionality, such as viewing notes and test results, state and local laws require authorization from the minor child (aged 12-17) To regain access to the medical record functionality please submit a signed authorization form: MyChart Proxy Access Authorization Form. (requires your child's signature)
NOTE: We require a method of payment on file. We do not store any credit card information on file in our office or on any of our computers. All transactions are processed through a secure gateway that adheres to all legal compliance requirements. Your financial security is our priority.
